Confusing disability insurance questions!?

I recently had surgery on my right for a removal of a lump (lumpectomy). This surgery occurred on Friday September 17th 2010. My employer only gave me 3 paid sick days and 1 paid personal day. This is how I'm getting payed for the week following my surgery due to my absence...Sun 19th and Mon 20th were my days off the 21st was my personal day the 22-24 are my 3 sick days. When I went to my doctor's office to schedule my surgery she gave me a note stating that I was allowed to return to work on the 25th. The note was faxed to my employer therefore I'm scheduled to return to work on the 25th. After my surgery the doctor told me she would be seeing me in 2 weeks. I called her office and they told me that I'm not to return to work until I see her. Because she's going to be out of town for a few days there are only 2 appointments available. One of them would be Thrus. Sept 30th and the other would be Tues. Oct. 12th. That would mean that I would be at a loss of pay starting the 25th of Sept. This is where this starts to get complicated. I need to have another surgery done by the end of this year. I need to have 3 wisdom teeth removed surgically. this would mean that I would be out of work for another week for sure. I'm thinking that if I attend my appointment on Sept. 30th I would for sure be released to return to work immediately. If I attend my appointment on Oct. 12th I could apply for Disability and serve my waiting period and collect a few days worth of benefits. I have an appointment set with my dentist on Sept. 30th. I'm wondering that if there are any immediate appointments available with my dentist to get my wisdom teeth removed if it would be possible to remain on disability and get this other procedure done as well. what is the most financially smarter thing to do. Keep in mind that I'm trying to make sure I don't miss out on money opportunities if that makes any sense. I like many other people have bills to pay lol And unfortunately I contribute the majority of the money to my family. Should I take the appointment on Sept. 30th and miss out on a week waiting period of disability and then when i get my wisdom teeth pulled out serve my waiting period then? or should I take the appointment on Oct. 12, serve my waiting period, collect some disability money and try to get my wisdom teeth pulled out before my doctor releases me to return to work so i can continue to collect disability money? please help!
